Job Description

  • Conduct comprehensive assessments to evaluate patient conditions and develop personalized care strategies.
  • Collaborate with interdisciplinary team members to deliver holistic care, fostering a supportive environment for patients and their families.
  • Provide hands-on nursing care, focusing on symptom management and enhancing patient comfort in their home settings.
  • Educate patients and families on disease processes, treatment plans, and self-care techniques to empower them in their care journey.

Qualifications:
  • Current Registered Nurse (RN) license in the state of New Jersey.
  • Experience in hospice or palliative care, with a focus on patient-centered approaches.
  • Ability to conduct comprehensive patient assessments and develop individualized care plans.
  • Proven track record of effective collaboration within interdisciplinary teams.
  • Strong communication skills to educate patients and families on care strategies and disease processes.
  • Proficiency in maintaining accurate and timely documentation of patient care activities.
  • Ability to manage multiple patient cases with compassion and precision in a field setting.
